LAST WILL AND TESTAMENT:

This is a document whereby you state how you want your estate to pass upon your death.  You will name a person or persons to act as your Personal Representative (Executor) who will oversee te wishes you set forth in your Will.  If there are minor children, you can designatre someone to be named as Guardian of your children (will oversee the care and general welfare of your minor child); you can designate a Conservator who who handle all financial matters of your minor child) and you can designate a Trustee who whil oversee your  child's assets after 18 years of age until an an age that you designate.

DURABLE (FINANCIAL) POWER OF ATTORNEY:

Document whereby you designate someone to handle your financial affairs while you are still alive if you are unable to make those decisions due to being mentally or physically incapacitated.

Health care power of attorney:

Document whereby you designate someone to handle your health care matters on your behalf while you are still living should you become mentally or physically incapacitated.

mental health care power attorney:

Document whereby you designate someone to be able to make mental health care decisions on your behalf  if you require inpatient behavioral health treatment in a Level 1 behavioral health facility.

LIVING WILL:

Document whereby you designate someone to make end of life decisions for you, i.e. if you wish to be taken off life support, etc., and not have your life prolonged in the event doctors have determined that there is no hope for your recovery.  It also directs whether or not you wish to be cremated or donate your organs.

beneficiary deeds:

Document that is prepared and recorded in the county where your real property is located whereby you name someone to inherit your real property only after your death.  The title to your real property will pass directly from you to the person named as the beneficiary on the Deed.  This is a means of avoiding probate.
